Illinois Owner's Sworn Statement under Mechanics Lien Act
Illinois property owners, the Mechanics Lien Act requires you to execute this Owner's Sworn Statement to protect the lien rights of all the subtrades and suppliers involved in a construction or renovation project on your property.
The Sworn Statement is made by the owner of the property, to give notice of all parties that have provided labor, materials or services with respect to improvements to the property. The form includes a schedule setting out the contract amounts, amounts paid to each contractor to date, and balance due.
