Illinois Survivorship Affidavit

Do you own property in Illinois jointly with someone who has recently passed away? You will need to file this Survivorship Affidavit in order to have the property transferred into the name(s) of the surviving owner(s).

The Affidavit sets out the details of the deceased’s passing and the value of the estate, and must be accompanied by a copy of the death certificate.
