UK Request for New Share Certificate

If you hold shares in a UK company, you can request the company to issue you a replacement for a share certificate that has been lost or destroyed by tendering this Request for New Share Certificate to the company's secretary.

The shareholder ⁄ member indemnifies the company against any claims or costs arising as a result of the company issuing the new certificate. The member agrees to return the replacement certificate in the event that the original is located.
