England & Wales Lasting Power of Attorney - Personal Welfare
Plan for the future and protect your interests, make a Lasting Power of Attorney (LPA) - Personal Welfare with this form for England and Wales. The form cannot be used until it has been registered with the Office of the Public Guardian.
As of October 2007, the LPA replaced the Enduring Power of Attorney, under the Mental Capacity Act 2005. An LPA allows you to name someone you trust to act as your attorney(s), to make health and personal welfare decisions for you when you are unable to do so.
This Lasting Power of Attorney - Personal Welfare was prepared by the Office of the Public Guardian, and is valid in England and Wales only.
